Argos president message
First of all let me thank you for your participation in this year’s ARGOS symposium on “Critical decision making in degenerative spinal stenosis”. Those of you who have already attended our meetings know by now that ARGOS symposiums’ major purpose is to enhance dialogue and to make it possible to share experiences with one another. The symposium proceedings used to be three half-days. This year’s topic requires more time for discussion. Therefore this year’s proceedings will be four half days, thus making it possible to have the most favourable conditions for exchanges of quality. As usual, clinical cases will be presented all along the symposium proceedings, allowing the discussion of details of the data which were examined during the previous session. They will be followed by a vote with the assistance of inter-active buzzers, making it possible for everyone’s convictions to be expressed accordingly. Last but not least, this year’s free communications sessions will be held both symposium days.

Thursday morning’s session will focus on diagnosis. Alan Crockard will share with us his important experience with myelopathy and will help us to assess clinically the different levels of cord compression. When should one consider a tandem stenosis ? What are the tests for quantitative evaluation of cervical myelopathy ? are the questions he will address in his conference on clinical diagnosis criteria for multi-level stenoses. Radiographic investigations will then be presented jointly by Jean Denis Laredo, neuroradiologist, and Christiane Strauss who will then give us an overview, describing the different contributions of each type of investigation to the diagnosis. Jean-Pierre Elsig will give the audience an insight into a novel imaging technique, functional MRI and will explain the potentialities of this new device and how it will change our investigation patterns. Electromyogram (EMG) and evoked potentials (SSP) and their relevance to differentiate the different involved vertebral levels will be discussed by Patrick Corlobé. Thursday morning’s session will end with the treatment strategy. Carlo Logroscino will address the various indication criteria serving to surgical decision.

Thursday afternoon’s session on cervical stenosis will start with Evelyne Emery and Paul Cooper presenting the decision criteria in cervical stenosis : treatment strategy, surgical approach and technique. Again, indications, surgical decision criteria, choices of different approaches and surgical protocols will be discussed. The second conference will be more of a debate : does laminectomy still hold a place in the treatment of cervical stenosis ? When is it necessary to add a fixation after laminectomy and/or laminoplasty ? Paul Cooper and Kasumasa Ueyama will give their points of view on this topic before opening the discussion with the audience. Spinal cord injury in cervical stenosis. What to do in the acute period versus at long term ? will be the last topic to be addressed on Thursday afternoon by Kasumasa Ueyama and Evelyne Emery. The authors will remind us the clinical symptoms and describe the way such diseases happen and what findings are demonstrated on MRI or CT. They will also teach us how to react in such cases both on a short time basis and on the long run.

Friday morning’s proceedings will cover the “thoracic stenosis” session. Stefano BORIANI and Stephan GAILLARD will present successively their viewpoints on treatment strategy : arthritic stenosis—soft hernia—calcified hernia. When should one operate ? What approach and technique ? They will give us their experience in the treatment of the different types of thoracic stenosis, when dealing with a soft disc versus a calcified disc or a multi level arthrosic compression. They will also mention the role of thoracoscopic approaches versus open surgery and present their results and complications.

“Lumbar stenosis” session will start the Friday afternoon’s proceedings. “Treatment strategy : when should one perform additional stabilization and/or fusion after posterior decompression ?” will be the first topic of the session. Jean-Pierre ELSIG and Stefano BORIANI will explain when one should perform a spine fixation, stressing the different situations when fixation and or fusion are mandatory.

The last session of the symposium will focus on complications. Neurological post operative complications (diagnosis, clinical presentation, investigations, treatment) will be presented by three speakers : Kasumasa UEYAMA, Stephan GAILLARD and Carlo LOGROSCINO. Each speaker will address a specific level of the spine focussing on the neurological complications knowing that some are more level specific while others are common to the different levels but the clinical presentation and investigations are different depending on the level. Likewise, the alternatives for complications treatment will be clearly stated.

As each year, a symposium synthesis will be made by Pierre KEHR who will give the attendees a take home message that will close the symposium proceedings. Last year’s farewell dinner at Bel Canto restaurant in Neuilly sur Seine met such a tremendous success that we could hardly imagine inviting you somewhere else. Therefore, we propose to those of you who attended the meeting last year to renew this marvellous experience and to the new attendees to discover the beautiful voices of young opera singers who will serve you while performing great airs of opera (Verdi, Mozart, Bizet, Rossini, Puccini…).

We are looking forward to welcoming you to this new edition of International ARGOS symposium. We hope that the scientific programme will be as rich as ever thanks to the quality of your input and that you will enjoy the magic moments of the farewell dinner.
